STEP 2

Once you find the perfect frames, simply place your order!  Email orders@jonaspauleyewear.com a copy of the prescription & PD measurement to complete your prescription order! 

STEP 3

Fill out an "out-of-network" form and mail it to your provider.

Click on the logo of your insurance provider below or download our simple out-of-network claim form to request reimbursement from your insurance company.
